We are Jo Mears and Sally Clarke and we would like to take this opportunity to re-introduce ourselves and the service we provide. As Special Schools Nurses we work alongside your School and the Professionals involved in your Child/Young Person’s care. We collaboratively cover Fountains Primary School, Fountains High School, Fountains Sixth Form, Loxley Hall School and Armitage.
As our team is ever expanding we now have an Outreach Team of School Nurses who provide cover over North & South Staffordshire Schools. This is on an adhoc basis so it maybe from time to time they contact yourselves on our behalf to ensure your concerns are addressed in a timely manner. Please be reassured that the Outreach Team feedback to ourselves.

Medication
For your child to receive medication in school we require:
· Up to date signed consent
· Prescription label to match consent
· Medication to be in date and sufficient stock
It is the parents / guardians / carers responsibility to ensure that all of the above is in place to ensure your child is well and safe within school.
Any prescribed medication to be administered three times a day will not be given in school.
When medication is being brought into School can we please ensure that it is handled by either parent / carer / guardian, transport escort or independent traveler (if your young person travels by public transport/walks) and is then to be handed over and signed in at Reception.
Dr's Clinic
We have a Community Paediatrician clinic held in school on a monthly basis.
If your child is under the care of the Community Paediatrician’s and you have a new concern let us know.
If your child doesn't currently see a Paediatrician and you have a concern in relation to their health, please get in touch to explore this!
Continence
All children / young people 5 years old and above are entitled to an assessment to be considered for a prescription of continence products.
You can ask ourselves if you would like an assessment to be completed.
All children / young people already receiving continence products will have a reassessment either 6 or 12 monthly dependent on products prescribed.
This reassessment may highlight that changes are required.
If you require a product change in-between the annual assessment or would like to discuss toilet training please contact ourselves.

Children and Young Peoples Autism Service
For any Child/Young Person that has a diagnosis of Autism that lives within Staffordshire has access to support with the Children and Young Peoples Autism Service. They will have open access should they require support. They can provide support for challenging behavior, sensory needs, anxiety and sleep. They provide a toolbox workshop that we would advise to access that look at these areas to provide advice/strategies. To book please contact the team via this email cypautismservice@mpft.nhs.uk
